Splenomegaly

 

Enlargement of the spleen may be due to:

§        Neoplastic Disorders

Chronic myeloid leukaemia, polycythaemia rubra vera, chronic lymphocytic leukaemia, lymphoma, myeloma

©       Inflammatory Disorders

Infectious mononucleosis, brucella, septicaemia, typhoid, tuberculosis, sarcoid, rheumatoid arthritis (Felty’s syndrome)

ª       Extramedullary Erythropoiesis

Myelofibrosis, thalassaemia

¨       Hypertrophy

Haemolytic anaemia (Hereditary spherocytosis), idiopathic thrombocytopaenic purpura

§        Congestion

Malaria (acute phase in the chronic phase hyperplasia occurs)

©       Portal Hypertension

Cirrhosis of the liver, portal vein thrombosis

 

Hyposplenism

Hyposplenism cannot be detected clinically. It may be caused by:

ª       Coeliac disease

Lysosomal storage diseases are a group of disorders caused by inherited deficiencies of individual enzymes within lysosomes. Patients with Niemann-Pick disease, Gaucher disease have hepatosplenomegaly.

Para-Aortic Lymph Nodes

 

If para-aortic lymph nodes are palpable consider:

©       Generalised Lymphadenopathy. Para-aortic lymphadenopathy may be part of a generalised lymphadenopathy

ª       Remember the drainage of the lymph nodes in particular testicular tumours in a male
